FLORIDA CITIZENS’ CREDENTIALING APPLICATION
ATTENTION: If you are NOT currently under another vendor’s contract with Florida Citizens and have six (6) months or more property claim handling experience, we invite you to follow the directions below to get credentialed through Pacesetter Claims Services. If you do not meet these requirements, be sure to come back and complete your application once you do.

Step 1. – GETTING YOUR DOCUMENTS READY TO UPLOAD

- To complete the Citizens’ Credentialing application, download the required documents: Citizens’ resume and Disclosure for Background Check. To download, chose one of the links below:
Please Note:
- To electronically sign your name on the Disclosure for Background Check, use the draw feature to sign your name.
- For information purposes only, click here to review full background check disclosure details.
- The Citizens Excel format resume has two tabs, which are located at the bottom of the Excel document: Designee Experience and Specialty Questionnaire. Be sure to review and complete both tabs. The Specialty Questionnaire ties into specific qualifications selected on the Designee Experience tab.
- Complete the Citizens’ resume and Disclosure for Background Check and save a copy ready to upload. Do not alter the Excel document format.
- Save a copy of your Florida self-appointed adjuster’s license ready for upload.
- Save a front copy of your state driver’s license ready for upload.
- Save a headshot photo ready for upload. This should be a professional image, like a passport picture.
Step 2. Complete the form below and upload required documents.
Please Note: All completed required documents must be attached to the form below: Citizens’ Excel resume (provided in unaltered format), background authorization, headshot photo, Florida adjuster’s license, and state driver’s license.
